Timeline

We’re excited to offer medical cannabis evaluations soon. To make the most of your certification—since it’s only valid for a limited time—we’ll begin scheduling appointments once dispensaries are open and patients can immediately access treatment.


Dr. Thomas Lewis, MD is qualified by the State of Alabama to certify patients for medical cannabis use under the Alabama Compassion Act.

What You Need to Bring to Your Appointment

To ensure a smooth and efficient visit, please bring the following:

1. Valid Photo ID

  • Driver’s license or government-issued ID

2. Medical Records (required) You may also email (admin@vulcanwell.com) or 

Fax: 833-799-3664

  • Documentation confirming your qualifying condition
  • Examples include: office visit notes, diagnosis records, imaging reports, or hospital records
  • Records should be recent and clearly state your diagnosis.

3. Current Medication List

  • Include prescription medications, over-the-counter medications, and supplements

Our Services

At Vulcan, medical cannabis is approached with the same level of care as any other treatment we offer—personalized, intentional, and rooted in medicine.


This is not a quick certification visit.  

It is a comprehensive medical evaluation designed around your health.


We will:  

- Review your medical records, symptoms, and goals  

- Evaluate whether medical cannabis may be appropriate for your condition  

- Discuss potential benefits, risks, and alternatives  


You’ll leave with a clear understanding of your options and next steps.

Drug Screening and Pregnancy Testing

To ensure your care is both safe and appropriate, we include routine medical screening as part of your visit.


This may include:  

- A urine screen  

- A pregnancy test, when appropriate  


These are simple, in-office steps that support responsible care and alignment with Alabama medical guidelines. 


Drug screening and pregnancy testing, when clinically indicated, are billed separately to ensure patients are only charged for services they actually need. 

Certification

Certification, if appropriate, is determined solely at the discretion of the evaluating physician in accordance with guidelines set by the Alabama Medical Cannabis Commission and the Alabama Board of Medical Examiners. Medical records diagnosing a qualifying condition will need to be provided at your appointment.


Only the following conditions qualify for medical cannabis certification per the Alabama Medical Cannabis Commission (AMCC):

 

  • Autism spectrum disorder (ASD)
  • Cancer-related cachexia, nausea or vomiting, weight loss, or chronic pain
  • Crohn's disease
  • Epilepsy or a condition causing seizures
  • HIV/AIDS-related nausea or weight loss
  • Panic disorder
  • Parkinson's disease
  • Persistent nausea that is not significantly responsive to traditional treatment (except nausea related to pregnancy, cannabis-induced cyclical vomiting syndrome, or cannabinoid hyperemesis syndrome)
  • Post-traumatic stress disorder (PTSD)
  • Sickle cell anemia
  • Spasticity associated with a motor neuron disease, including amyotrophic lateral sclerosis (ALS)
  • Spasticity associated with multiple sclerosis or a spinal cord injury
  • Terminal illness
  • Tourette's syndrome
  • A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following: cachexia or wasting syndrome; chronic pain; severe or intractable pain that has not responded to adequate trials of other treatment modalities; nausea or vomiting; seizures; severe and persistent muscle spasms


We do not dispense or sell cannabis products.

Appointments

All appointments must be in person and the patient must be a resident of Alabama.


Follow-up appointments:

Every 30 days for patients capable of conception

Every 90 days for all other patients
