state-regulated certifications-coming soon

Timeline

We’re excited to offer Alabama's alternative medicine evaluations soon. To make the most of your certification—since it’s only valid for a limited time—we’ll begin scheduling appointments once dispensaries are open and patients can immediately access treatment.

Dr. Thomas Lewis, MD is qualified by the State of Alabama to certify patients under the Alabama Compassion Act.

Our Services

At Vulcan, this alternative medicine is approached with the same level of care as any other treatment we offer—personalized, intentional, and rooted in medicine.

This is not a quick certification visit.

It is a comprehensive medical evaluation designed around your health.

We will:

- Review your medical records, symptoms, and goals

- Evaluate if this is appropriate for your condition

- Discuss potential benefits, risks, and alternatives

You’ll leave with a clear understanding of your options and next steps.

What You Need To Send Prior to the Appointment

Medical Records (required) You must E-mail admin@vulcanwell.com or

Fax 833-799-3664 your medical records at least two (2) business days prior to your appointment to avoid a possible cancellation.

  • Documentation needs to confirm your qualifying condition (see list below of qualifying diagnoses)
  • Examples include: office visit notes, diagnosis records, imaging reports, or hospital records
  • Records should be recent and clearly state your diagnosis
  • A $35 medical records review fee is non-refundable once the appointment is made.

What You Need to Bring to Your Appointment

To ensure a smooth and efficient visit, please bring the following:

1. Valid Photo ID

  • Driver’s license or government-issued ID

2. Current Medication List

  • Include prescription medications, over-the-counter medications, and supplements

Drug Screening and Pregnancy Testing

To ensure your care is both safe and appropriate, we include routine medical screening as part of your visit.

This may include:

- A urine drug screen

- A pregnancy test, when appropriate

These are simple, in-office steps that support responsible care and alignment with Alabama medical guidelines.

Drug screening and pregnancy testing, when clinically indicated, are billed separately to ensure patients are only charged for services they actually need.

Certification, if appropriate, is determined solely at the discretion of the evaluating physician in accordance with guidelines set by the Alabama MC Commission and the Alabama Board of Medical Examiners.
